Form Builder — Create Custom Client Intake Forms

FlowEdge’s Form Builder lets you create custom intake forms that collect exactly the information you need from clients. No coding required — just pick your fields, arrange them, and share.
Getting Started
- Click Forms in the sidebar.
- Click Create Form.
- Give your form a name (for example, “Website Project Intake” or “New Client Questionnaire”).
The 8 Field Types
The Form Builder offers eight types of form fields:
Text
A single-line text input. Use it for short answers like names, company names, or job titles.
Textarea
A multi-line text area. Perfect for longer responses like project descriptions, goals, or special requirements.
Select (Dropdown)
A dropdown menu with predefined options. Great for choices like “What is your budget range?” or “What industry are you in?”
Radio Buttons
A set of options where the client picks one. Similar to a dropdown but all options are visible at once. Use when you have 2-5 options.
Checkbox
A toggle that the client can check or uncheck. Good for yes/no questions like “Do you have an existing website?” or for selecting multiple options from a list.
Date
A date picker. Use it for timeline questions like “When do you need the project completed?”
An email input with built-in validation. Ensures the client enters a properly formatted email address.
Phone
A phone number input. Collects the client’s phone number in a standard format.
💡 Tip
Start with 5-8 fields. Too many questions can overwhelm clients and reduce completion rates. You can always follow up for more details later.
Configuring Fields
Each field has several options:
- Label — The question or prompt the client sees (for example, “What is your project budget?”)
- Placeholder text — Example text that appears in the field before the client types (for example, “$5,000 - $10,000”)
- Required toggle — Mark a field as required so clients cannot submit the form without answering it
Reordering Fields
Drag and drop fields to rearrange their order. Put the most important questions first and group related questions together.
✅ Best Practice
Start with easy questions (name, email) and build up to more detailed ones (project scope, budget). This eases clients into the form and improves completion rates.
Sample Intake Form Structure
Here is a starter template for a consulting intake form:
- Full Name (text, required)
- Email Address (email, required)
- Company Name (text)
- Project Description (textarea, required)
- Project Timeline (select: “1-2 weeks,” “1 month,” “2-3 months,” “3+ months”)
- Budget Range (select: “Under $5K,” “$5K-$10K,” “$10K-$25K,” “$25K+”)
- How did you hear about us? (text)
- Additional Notes (textarea)
